      OBJECTIVE: To probe into the effect of Dangua Humai oral liquid on vascular endothelial function and oxidative stress indicators in patients with type 2 diabetes mellitus (T2DM) complicated with non-alcoholic fatty liver disease ( NAFLD) under the guidance of co-treatment of phlegm and stasis. METHODS: Totally 92 patients with T2DM complicated with NAFLD admitted into the Affiliated People's Hospital of Fujian University of Traditional Chinese Medicine from Jan. 2018 to Jul. 2020 were selected and divided into control group ( 46 cases) and observation group ( 46 cases) according to the admission sequences. The control group was given conventional therapy, while the observation group was given Dangua Humai oral liquid based on the control group under the guidance of co-treatment of phlegm and stasis, both groups were treated for 3 months. The clinical efficacy, blood glucose [fasting blood glucose( FBG), 2 h postprandial blood glucose ( 2 hBG) and glycosylated hemoglobin ( HbA1 c) J levels, blood lipid [ total cholesterol ( TC), triglyceride ( TG), high-density lipoprotein ( HDL ) and low-density lipoprotein ( LDL) ] levels, liver function indicators [alanine aminotransferase (ALT), aspartate transaminase ( AST) and y - glutamyltransferase ( GGT) J, vascular endothelial function indicators [vascular endothelial growth factor ( VEGF ), endothelin (ET-1) J and oxidative stress indicators [malondialdehyde (MDA), superoxide dismutase ( SOD) J levels were compared between two groups before and after treatment. RESULTS: The total effective rate of observation group was 89. 13% ( 41/46), which was significantly higher than that of the control group ( 71. 74%, 33/ 46 ), with statistically significant difference ( P <0. 05) . After treatment, the FBG, 2 hBG, HbA1 c, TC, TG, LDL, ALT, AST, GGT, ET-1 and MDA levels of both groups had been significantly decreased, while the HDL, VEGF and SOD levels were significantly increased, with statistically significant differences (PO. 05 ), the 2 hBG, TC, TG, LDL, ET-1 and MDA levels of observation group were significantly lower than those of the control group, while the HDL, VEGF and SOD levels were significantly higher than those of the control group, with statistically significant differences ( P < 0. 05) . CONCLUSIONS: Under the guidance of co-treatment of phlegm and stasis, Dangua Humai oral liquid can better regulate blood lipid and blood glucose metabolism in patients with T2DM complicated with NAFLD, improve their vascular endothelial function, reduce oxidative stress and promote therapeutic effect, without significant adverse drug effects.       目的:探讨痰瘀同治指导下丹瓜护脉口服液对2型糖尿病(type 2 diabetes mellitus,T2DM)合并非酒精性脂肪性肝病(non-alcoholic fatty liver disease,NAFLD)患者血管内皮功能及氧化应激指标的影响。方法:选取2018年1月至2020年7月福建中医药大学附属人民医院收治的T2DM合并NAFLD患者92例,按照入院先后顺序分为对照组46例和观察组46例。对照组患者给予常规治疗,观察组患者在对照组的基础上根据痰瘀同治指导理论予以丹瓜护脉口服液,均治疗3个月。比较两组患者的疗效,治疗前后血糖[空腹血糖(FBG)、餐后2 h血糖(2 h BG)及糖化血红蛋白(Hb Alc)]、血脂[总胆固醇(TC)、甘油三酯(TG)、高密度脂蛋白(HDL)及低密度脂蛋白(LDL)]、肝功能[丙氨酸转氨酶(ALT)、天冬氨酸转氨酶(AST)及γ-谷氨酰转移酶(GGT)]、血管内皮功能[血管内皮生长因子(VEGF)、内皮素(ET-1)]及氧化应激[丙二醛(MDA)、超氧化物歧化酶(SOD)]等指标水平。结果:观察组患者的总有效率为89.13%(41/46),明显高于对照组的71.74%(33/46),差异有统计学意义(P<0.05)。治疗后,两组患者FBG、2 h BG、Hb Alc、TC、TG、LDL、ALT、AST、GGT、ET-1及MDA水平较治疗前明显降低,HDL、VEGF及SOD水平明显升高,差异均有统计学意义(P<0.05);观察组患者的FBG、Hb Alc、ALT、AST及GGT水平与对照组的差异均无统计学意义(P> 0.05),2h BG、TC、TG、LDL、ET-1及MDA水平明显低于对照组,HDL、VEGF及SOD水平明显高于对照组,差异均有统计学意义(P<0.05)。结论:痰瘀同治指导下使用丹瓜护脉口服液治疗,能够更好地调节T2DM合并NAFLD患者的血脂和血糖代谢,改善其血管内皮功能,减轻氧化应激反应,提高治疗效果,且无明显不良反应.
