      The article discusses the birth of the Rapid Evaluation and Care at Home (REaCH) comprised of a specialist team to address primary and mental health and contain alcohol and drug use in Canberra, Australia in 2021. It cites the goal to limit coronavirus positive Emergency Department presentations and hospital admissions, and prevent deterioration within quarantined homes. Also noted is the collaborative work among nurses to support people with comorbid conditions and create positive outcomes.
